Parish Ministry Forms:
- Room Reservation Form: To request the use of a church facility, the Room Reservation Form must be filled out and submitted to the parish office prior to scheduling your event. NO PHONE REQUESTS WILL BE ACCEPTED.
- Facility Work Order: Please submit all requests for maintenance or custodial work by email to: grunkles@stclementpc.org.
- Request for Youth Participation: Please submit all requests to Myriam Reyna at least 1 month prior to event to: mreyna@stclementpc.org
