Parent/Guardian Consent

I consent to my child being registered in the St. Clement Catholic Church Confirmation Program as administered by the parish Faith Formation Department. i understand in this program, Christianity is presented according to the teaching of the Roman Catholic Church.

I understand that as the primary catechist for my child, I assume responsibility for their attendance, behavior, and meeting the guidelines for the preparation of this sacrament.
